What does S mean in UM02?

S (Revised): A request to revise an existing certification — the service, dates, provider, or quantities originally certified have changed and the certification needs to be modified to match what will actually occur.

How S is used

Sent with the original certification reference plus the corrected details. Distinct from a renewal (more of the same) and an extension (more time): a revision changes the substance of what was certified.

Example

A certified outpatient procedure is moved to a different facility; the provider submits UM02 = S so the certification reflects the new site of service.
