Sent with a reference to the existing certification so the UM organization can link the renewal to the original determination. Common for ongoing therapies, home health episodes, and recurring services.
Example
Twelve authorized PT visits are used up and the patient needs more: the provider submits UM02 = R referencing the original authorization number.
