Discretion is narrower than repeal: the 278 remains the adopted standard, any covered entity can still require it, and the discretion covers end-to-end FHIR exchanges — not mixed flows. Practical consequence: dual-rail architectures (FHIR at the edges, 278 where partners require it) remain the realistic operating model for years.
