PAS was designed to coexist with HIPAA: its data model deliberately maps to 278 semantics so intermediaries can translate between rails. HHS enforcement discretion (announced with CMS-0057-F) means end-to-end FHIR PAS exchanges won't be penalized for skipping the X12 conversion — but any party can still require the 278, so most architectures translate rather than replace.
