How 75 is used
Returned in AAA03 at the subscriber loop. Confirm current coverage and correct payer routing via 270/271 before resubmitting; repeated 75s to one payer can indicate a routing table error.
Example
A request routed to a member's prior carrier after a January plan change returns AAA03 = 75.
