Returned in HCR01 on the 278 response at the level (patient event or service) being answered. In CMS-0057 metrics terms, these responses feed the approved-request counts payers must publicly report.
Example
A request for 12 physical therapy visits comes back HCR01 = A1 with a certification number and the authorized visit pattern.
